DNV GL has been helping clients for many years with incident investigations using its root cause analysis tool SCAT. In that time DNV GL has expanded on SCAT to create an incident investigation methodology focusing on barriers. This methodology allows incidents to be investigated regardless of size or complexity, to delve through immediate and basic causes to expose the underlying management system failures.
